Online Account Management Effective June 1st, advanced website tools are just a click away at www.chcflorida.com

Account Management is a tool designed specifically for employer groups to help simplify plan administration. The tool makes it easy to:

* view up to six months of group statements and invoices
* view payment history and current account balances
* view eligibility information for any employee and find the invoices on which that employee has appeared
* view a roster of all employees and their covered dependents
* add or delete a dependent to an employee’s coverage
* perform a variety of functions on behalf of any covered employee or dependent, including requesting a member ID card and updating an address or phone number

Other functions include:

* make online payments, which will be available shortly after the system’s June launch date
* helpful, easy-to-use reporting features for groups with over 200 enrolled employees

New Invoicing

If you receive a monthly bill for your group we’re making enhancements to your invoice. In June you will receive an invoice, for your July premium, that features new information and is easier to understand. To familiarize you with the format of the new invoice, a sample is below.

1. This section contains your group and invoice number – this information is important and helps us process your payments correctly. Each bill will contain a payment coupon. It is necessary for you to use your payment coupon when mailing your payment. However, if you do not use the coupon, be sure to reference your group and invoice number – this will ensure the accurate and timely processing of your bill.
2. This section contains the address that you should use when mailing your premium payments.
3. Account Summary – summary of prior transactions (payments and/or adjustments) to your account since the last invoice period.
4. New Charges for Coverage Period – premium charges for the current coverage period.
5. Retroactive Charges for Coverage Period – premium charges/credits for the prior periods that have not been billed to you or included on previous invoices.
6. Current Monthly Charges -The total monthly charges for the particular coverage period (#4 and #5 combined).
7. Account Balance Payment Due the amount you owe, which includes any unpaid balance reflected in section #3 plus your current monthly charges.

Effective June 1st, Medco replaces our current pharmacy benefit manager (PBM).
We are excited about Medco and look forward to providing our members with all the benefits that come from using a PBM that is dedicated to operational excellence, customer engagement and superior, customized services. We have many processes in place, to ensure that members experience a smooth transition.
What will change?
•Mail-order provider – Medco will be the new mail-order provider. All available refills on mail-order prescriptions will be transferred to Medco. Members currently using the mail-order service will be notified that their prescriptions are being transferred. They will also receive new forms and instructions on how to submit prescriptions to Medco.
•ID cards – members will receive a new ID card in May. The new card, which will be effective June 1st, will contain information essential for pharmacies to accurately process claims.

New Member ID

Cards Towards the end of May, you and each of your covered dependents will receive a new member ID card with the Coventry Health Care of Florida name. The ID card, which will be effective June 1st, will also contain important information that you will need to access health care services, including: • New member ID number • New group number • Customer service telephone number • New pharmacy benefit manager logo and information Please review the card carefully and call Customer Service if corrections are needed. The phone number is on the front of your ID Card. Be sure to discard your old ID card and beginning June 1st, provide the new card to your health care providers and pharmacy.

In most cases, when you or a covered dependent access health care services, you receive an Explanation of Benefits (EOB). The EOB explains how your medical claims, including payments and denials, are processed. An Explanation of Benefits is not a bill. You should always review your EOB to make sure that you received the services for which your health plan is being billed.
To help familiarize you with the format of an EOB, we have included a sample below.

1. The address to which the EOB was mailed.
2. Group Name – the payor of your medical claim. If your company is a self-funded group, their name will appear here.
Insured – the person who holds the contract with the insurer.
3. Patient – the person who received medical services. This may be a subscriber or a dependent.
4. ID Number – the identification for the person receiving medical services.
5. Claim Number – document control number generated by Coventry Health Care. If you need to call a member service representative to discuss the claim, this is an important number to give them.
6. Provider – the provider of your medical service. This could be an individual practice or facility.
7. Member Responsibility – this is the amount the member is responsible to pay the provider.
8. Date – the date your medical services were incurred.
9. Procedure Code/Description – the coverage category for which the code is classified.
10. Billed Amount – the total amount billed to Coventry Health Care by your physician.
11. Contractual Adjustment – reductions in payment due to network savings, coordination of benefits, or non-covered services. For more information, see number 19, Cont. Rmk/Other Rmk.
12. Approved Amount – the amount Coventry agrees to pay the provide for services rendered minus copays, coinsurance or deductibles, if applicable.
13. Less Copay
14. Less Coinsurance
15. Less Deductible
16. Less Other Amounts
17. Plan Paid – the amount paid by your plan.
18. Cont. Rmk/Other Rmk – a Coventry Health Care code that explains why certain amounts were not covered.

So now that health reform has passed and has been written about and reported on so extensively, the only topic left to cover is what will happen to health insurance brokers after health reform begins to change health insurance in roughly six months, and to their individual health insurance market. I am about to give a scenario which is based on the current law (yes health reform is now a law), and what has happened in states like New York and Massachusetts where this model originates.

By the way the quote button above is a reminder that you can still buy health insurance as an individual, which means that this massive website that I built can still be used to find affordable health insurance, until such time as it is replaced by the state health insurance exchange should this actually end up happening.

In my opinion (which is based on the General Accounting Office numbers which probably also qualifies as an opinion), individual health insurance will eventually end, at least in terms of agents, just like it has in Massachusetts. where everyone buys insurance from the exchange. So again, individual health insurance will still be available, but only in health insurance exchanges where each state is responsible for their own exchange and which companies offer plans through it.

For our current jobs in individual health insurance to stay viable, everyone would need to do as the law intends, which means buying health insurance or getting it through their employer. The rest of course will either qualify for premium assistance to make up the difference or Medicaid which is slated to expand to cover 35 million more Americans. In and of itself, this is a fallacy, as the state budgets are already so bankrupt and deficit ridden that they have been cutting benefits and coverage to people that really need it since the recession began. In fact Arizona just got rid of their CHIP program (health insurance for children) and would have cut even more, had this law not passed which specifically demands that the states not cut any programs nor make it more difficult to qualify for government health insurance programs.

Further, this new health care law is based on the absurd notion that younger and healthier people will buy health insurance through the health insurance exchange. This will inevitably not happen because premiums for younger Americans are going to surely become, at least in the short term even more expensive.

This is because the health care legislation will leave insurance companies with no other choice but to raise premiums even higher because for one, this law has provisions that keep premiums for the young and old within a certain ratio. Secondly, and more importantly, because the young and healthy simply will not ever think about health insurance much less buy it no matter what the law. Not to mention that health care costs have risen wildly on their own accord just in the time it took you to read this article.

And finally and most importantly, young people and most of the uninsured right now for that matter, will not buy health insurance because the law only penalizes them $695 which is not even significant when compared to what premiums are and what they will become under this law.

“..younger, healthy Americans likely will shoulder premium hikes, at least initially,” said industry lobbyist Kelly Atkinson, executive director of the Utah Health Insurance Association.

Most Americans, probably don’t realize this, but in New York for example if you shop individual plans right now, the cost for an 18 year old male is the same as a 64 year old with diabetes and any other condition you can think of. And this premium is currently about $1000 per month for both the 18 and 65 year old to get only average benefits.

Under this new law, the only people buying health insurance will be people that have insurance now (though many of them will elect instead to pay the penalty, I know I probably would) and everyone that has been declined. The rest will head towards Medicaid and to their employer.

The other nail in the proverbial coffin for health insurance brokers is the 80% rule, which simply states that 80% of premium must go to health care costs. This will eliminate most of the room to pay us even a reasonable commission as brokers after factoring in administrative costs.

Sadly for us this law takes effect as soon as 2011, which will in effect either put us out of business right away or dramatically hurt our bottom line. What is strange is that the health insurance exchange doesn’t become law until much later, which means that most health insurance brokers will be forced out due this 80% rule in the very near term. East Coast Health Insurance as well as some of our competition might be able to stay afloat longer because of the fact that we are larger producers and thus have bargaining strength.

Still, the 80% rule is reasonable and will help stop some of the waste that has polluted our industry, but unfortunately the bulk of the health care cost drivers will still remain, and will eventually poison the entire system with or without health reform. As one looks at the final bill that passed, I am not only amazed at the new problems that it will cause, but also and more importantly, it might be very well be the financial death blow to this economy.

A recent study found that administrative expenditures in the United States were four times higher than reported by NHE in 1999, or 31 percent of total health care spending, compared with only 17 percent in the predominantly government-run Canadian system. But this includes Medicare as well, so administrative expenses associated with private insurance could even be higher then 31%! You do the math.

This article is about the future of individual health insurance brokers, but still I will share my solution. My solution has no basis in the near term reality of the situation however, and will do nothing to stop the quickening demise of the individual health insurance agency.

But the best bet for our economy is to replace the 2400 page document with a 10 page document that sets up a single payer system while at the same time, leaving people the option to purchase private health insurance much like Europe. Everybody wins here, and more importantly it makes our health care system become a significantly less important piece of our gdp. In the end though however maybe in 10 years, that is what this new law will cause because health insurance companies will be unable to make enough money to rationally survive. I am quite certain that the people who wrote this bill know that as it seems like this bill will cause enough pain to force us into a single payer system.

Americans Want Health Reform, and I want a nap. But there is just not enough time budgeted for during my hectic day. Heck, in Japan they have observed that worker effeciency is increased dramatically with just a quick nap after lunch.

With the big health reform summit scheduled for tomorrow, the only thing that both companies, I mean political parties can agree on is that the health reform summit will be like trying to feed a dead bulemic. There will be no results. This will means for the Democrats another enormous political defeat and for Republicans another chance to ruin our economy by giving tax breaks to the wealthy.

Health Insurance Clauses, Limitations, and Exclusions Broken Down

Ok so I wanted to write a page about the exclusions and limitations in a health insurance policy. So in a totally new, never before done article I wanted to illustrate some of these exclusions and discuss them line by line. I chose Cigna because it was the first one that I picked randomly, and I am certainly not picking on Cigna as everyone else’s plans are virtually identical, and literally filled with the same exclusions and limitations. It is more an attack on the health insurance industry and I am not vindictive at all towards Cigna.

In any event lets look at some of these plan exclusions and discuss them. Feel free to email us and give us some suggestions and share your experiences with these individual or even group health insurance exclusions.

Individual and Family Plans EXCLUSIONS AND LIMITATIONS Conditions which are pre-existing as defined in the Definitions section.—This is pretty obvious, if you have an existing condition, the health insurance companies simply won’t cover it for as long as they can get away with it, and if it is a chronic condition you will most likely be declined for coverage all together. Most states only allow for a one year preexisting condition. But in many cases and states the health insurance companies will end up excluding the condition entirely which is called ridering a condition. I find this entire situation gross and unfair toward a segment of the population that needs assistance the most.

Day 2 | Diary of a Health Insurance Agent in Training

After walking into the online store of www.echealthinsurance.com and finding the people that run this place, I am glad to see the people that work here actually care about the consumers and are helping the uninformed to find the best possible solution (insurance) to their problems. I was a little overwhelmed as I walked into a place that seemed to be growing out of their office. Literally, people are fighting for desks if someone is out sick, but everyone seemed to be a happy family.

My first day, I got to sit in on a face-to-face sales call and watch someone save over $300 a month and receive a better policy! I just tried to sit back and watch, but my natural sales mentality made me jump in and start asking questions about the lady’s policy and why she wanted to change. Basically, she was paying too much, and wanted to see if there were better options out there. Of course, there were and Caroline ended selling the policy, but it did not really seem like she was selling her at all. That is what is great about what I witnessed. Being able to listen to a knowledgeable health insurance agent find out why someone is looking for new coverage, and just giving the consumer all the options she has is like going to a 5 star restaurant with a menu in a different language. You know everything on the menu is going to be great, but you have no idea what is going on until the waiter starts explaining the menu for you. Each product has its ups and downs, you may pay more for the 10 course meal, but at least you finally get an understanding of what you are about to receive.

I say this for the fact that even after getting licensed I have no idea how to sell health insurance, let alone buy it. So after asking 100 plus questions today, I came to the realization that I need to do my homework. The great peice of information and the reason I chose to work here versus a call center is that the agents I am working around aren’t pushing the fish that is going bad because they need to make money off of it (sorry for the bad analogies, I am new to blogging and was also in the restaurant industry for quite sometime), but because they want their potential clients to receive the best policy no matter which carrier pays the most or least. Well that’s it for my first real day… I already feel like I have cameras on me and I am fist pumping at the Jersey Shore…

Even if I do not know all the answers, I want to let them know I am going spend the time to find out those answers. If it matters to them, it matters to me.

Aetna Underwriting Update for Individual Policies | Health Insurance News, Views, and Reviews

Individual health insurance policies in the Florida health insurance market do not automatically include maternity benefits, but rather only cover any complications due to pregnancy. In order to receive any coverage for routine maternity benefits you would need to enroll in a medical plan that offered maternity coverage as an optional benefit since Aetna does not offer maternity benefits.

Health Reform Legislation Being Threatened | Health Insurance News, Views, and Reviews

The Republican Attorney Generals are threatening to make the health reform bill more difficult, though many in Washington are unsure of how that could even be possible at this point. According to the Associated Press, the Republican Attorney General’s from various states, are in a huff due to the extortion carried out by Nebraska’s Senator Nelson. At least 13 states are insisting that Congressional Leaders remove the unfair Medicaid referendum being promised to Nebraska in return for their support of the Senate Health Reform Bill.

The letter of distraught starts out with, “we believe this provision is constitutionally flawed,” which was written last Wednesday (I was busy working on our health insurance site so I apologize for the tardiness of this post).
